What You Need to Know

 

Our team of professionals help make the admission process a smooth transition. We know that entering a nursing facility yourself or admitting a loved one can seem extremely overwhelming. We recommend you visit and take a tour of the facility before you or your loved one is admitted. Our admissions team will answer any questions you may have and explain the admission process to you.

What to Bring With You Upon Admission:

The following information should be provided prior to or at the time of admission:

Personal Information:

  • Patient’s/Resident’s current address and phone number
  • Address and phone numbers of family or friends in case of emergency
  • Advance Directives (Power of Attorney, Living Will, Health Care Surrogate, etc)
  • Drivers License or Photo ID
  • Social Security Card

Financial Information:We strive for all of our patients/residents to be aware of and receive all the health care benefits that are available to them. Our team of professional finance experts are here to help you through any billing and/or application processes that you may need assistance with. If you are not utilizing insurance for your stay, private payment is due prior to admission. For your convenience we offer on-line credit card payment. Simply go to our home page and click on the “Pay Bill Online” link. You can help to expedite the processes by providing the following information before or at the time of admission:

  • Insurance Cards – Medicare, Medicaid, Managed Care, or Private Insurance
  • Medicare Part D card (prescription drug benefit)
  • Financial Power of Attorney (if applicable)

If you will be applying for Medicaid, please include the following:

  • Copy of Medicaid application if already completed
  • Birth Certificate
  • Proof of income
  • List of assets (bank accounts, CD’s, annuities, stocks, bonds, real estate, etc)
  • Value of life insurance
  • 3 months worth of bank statements
  • Copy of burial policy

Personal Items:

  • 1 week worth of comfortable clothing
    • Pants, slacks, shorts
    • Shirts, blouses
    • Undergarments
    • Socks
  • Rubber soled shoes (preferably sneakers)
  • Pajamas/Night Wear
  • Coat, jacket, or sweater
  • Book, pictures, writing supplies
  • Toiletries (if you prefer your own brands)

What Not to Bring With You:

  • Money
  • Valuables (jewelry, watches, etc)
  • Electronics
  • Irreplaceable family mementos
  • Original copies of important personal papers
